If you have a complaint about your health or dental benefits or coverage, please contact your insurance company directly. You can also contact the Office of the Insurance Commissioner to file a complaint about your health care benefits, coverage, or insurance company.

If something doesn’t seem right on your 1095-A, you can fill out a correction request or contact the Washington Healthplanfinder Customer Support Center at 1-855-923-4633. To fill out a correction request, visit 1095-A Correction Requests.
